INTRODUCTION
The formation of Calantas Young Dreamers Foundation, Inc.(CYDFI Philippines) into a legal entity in the
USA as a foreign nonprofit corporation shall be registered in the State of California as:
CYDF USA and shall
seek to be granted an IRS 1023 Tax-Exempt Status under the 501.c3 tol maximize and facilitate limited
resources generated from the Fil-Am community and the mainstream society to help accomplish the mission
of "empowering indigent but deserving talented youth so he can help other youth through a scholarship
program model". This definitive mission is shared by both the CYDFI Philippines and CYDFI USA.
However, CYDFI USA shall address its two other major missions defined in objectives 2 and 3, as declared
by the Founders and adopted by the Calantas Folks residing in the USA, manifested by the interim
documents of the Foundation: (1)
Articles of Incorporation and (2) By-Laws.

MISSION STATEMENT
CYDFI USA shall enable CYDFI Philippines to accomplish its mission in helping to break the cycle of
poverty among the youth in Philippine rural areas thru a Scholarship Program Model; design Community
Youth Empowerment Programs for Fil-Am "high-risk" in SF Bay Area; establish an ethnic-specific nonprofit
medical model healthcare facility for the Fil-Am aging population in SF Bay Area and advocacy to replicate
model in other parts of the country.
OBJECTIVES OF CYDF USA
In order to carry out its mission, CYDFI USA shall have the following objectives to
guide its methodology and conduct of business operation.
  1. To support the mission of CYDFI Philippines in "empowering indigent but deserving talented youth living in abject poverty found in Philippine rural
    areas like Bo.Calantas and Nabuclod Aeta Community located in Floridablanca, Pampanga through a Scholarship Program Model with a methodology
    called SWREM Theory In Action that is designed to help them help themselves so they can help incoming scholars leading to breaking their cycle of
    poverty as they learn to become socially responsible with implications, not only to personal but community growth and development; and, to promote
    replication advocacy of said Model in other communities beginning with Benedicto, another barrio in Floridablanca. (Bo. Benedicto is formerly known
    as Bo. Stable).
  2. To design Community Youth Empowerment Programs (CYEP) that promote positive self-image, enhancement of self-esteem, development of life and
    leadership skills of the Fil-Am "high-risk" youth and their families in the San Francisco Bay Area,
  3. To establish a nonprofit medical model healthcare facility, imbedded with ethnic-specific psycho-scocio-emotional, religious and culturally-sensitive
    services for the Filipino-American aging population in the San Francisco Bay Area and by advocating for replication of the model to other places in the
    country.

The first objective of CYDFI USA is a short-term one, designed to directly support the mission of the CYDFI Philippines and that which is envisioned to
become, in due time, a
self-sustainable institution, by seeking to employ "SWREM Theory In Action" as the key principle for its methodology. The last
two objectives are long-term which are designed to serve the Filipino-American "high-risk" youth and their immediate families and the aging population in the
San Francisco Bay Area and by advocating for replication of the model in other places of the country where there is a general tendency of higher Filipino
population as indicated by the United States Census Bureau.


METHODOLOGY
CYDF USA is organized with a mission espoused by CYDFI Philippines of empowering indigent but deserving talented youth of Calantas and Nabuclod
through a Scholarship Program Model with SWREM Theory In Action as the key concept for its methodology, with implications not only to personal but
community growth and development leading to breaking their cycle of poverty as they learn to be socially responsible. The former differs from the latter with
its mission of designing Community Youth Empower Programs (CYEP) and establishing a Geriatric Healthcare Center for Filipinos (GHCF).

CYDFI USA will have three (3) Phases of Program Development to serve as parameters in the implementation of the methodology to accomplish its mission
and objectives. Specifically, the following are designed to define the direction of the Foundation:
  1. The issues that CYDFI USA has identified to address and why?
  2. The programs adopted to effectively respond to the identified issues;
  3. The policy and procedural statements that would define the structures of the programs;
  4. The mission and objectives of the Foundation in responding to these issues.


DISCUSSION
I. ISSUES DEFINED BY CYDF USA
The Foundation shall attempt to respond and address the following issues as an integral part of its mission:

1.1. Breaking the Cycle of Poverty through a Scholarship Program Model
CYDFI USA recognizes the need to support the mission of CYDFI Philippines in breaking the cycle of poverty among the indigent but talented youth in
Calantas and Nabuclod through a Scholarship Program Model with SWREM Theory In Action, as a key concept for its methodology.
Rationale: The Founders believe that the "future belongs to the youth and the youth holds the light of the future with ardent passion to make a difference and
to effect change in his life for the better and those around him as he learns to become more socially responsible". For this dream to come true, youth needs
some resources to empower him become self-empowered, especially those indigent but talented youth in Calantas and Nabuclod through a Scholarship
Program Model. This model is designed to make the Foundation become a self-sustainable institution with the implementation of SWREM Theory In Action,
having implications not only to personal but community growth and development.The model is realistically feasible because of the continuous spiral loop
dimension that keeps the process on the move. Meaning, once scholars shall have gained momentum in number and begin to pour in their energies of "helping
one another", growth of scholarship eventually takes place and development takes effect in the community.

1.2. Community Youth Empowerment Programs (CYEP)
CYDF USA recognizes the need to develop and design Community Empowerment Programs that promote positive self-image, enhancement of self-esteem,
development of life and leadership skills of the Filipino-American "high-risk" youth in the San Francisco Bay Area.
Rationale: The Founders believe in the pro-active approach to make a difference in the life of youth so he can be more productive member of the society as
he gains more positive self-image with enhanced self-esteem and able to develop his leadership potential into a meaningful and useful one. When a youth
experiences the power of being self-empowered, he will one day become a "self-empowered" and can be a person who and what he wants to be and become,
enabling him to empower others, as well. This is the "ripple effect" of empowerment. It can spin things around and can spell a difference to effect a positive
change in other people's lives in the community where the youth lives in.

1.3. Geriatric Healtcare Center for Filipinos (GHCF)
CYDF USA recognizes the absence of and the need to establish a non-profit medical model healthcare facility for the Filipino-American aging population that
addresses the ethnic-specific psycho-socio-emotional, religious and culturally-sensitive needs of the individual deemed and bound for an institutionalized
medical healthcare setting.
Rationale. Per US Census 2000 Brief Report on the Asian Population, there are 1,850.314 Filipinos residing in the United States, and yet no known
establishment of institutional medical model healthcare facility for the Filipino-American
aging population with focus on ethnic-specific psycho-socio-emotional, religious and culturally-sensitive services is in
existence with a nonprofit status; nothwithstanding the fact that most Filipinos are acculturated and assimilated into the mainstream society but are still very
much Filipinos at heart and soul. Meaning, they still long to be with their cultural heritage, especially as they reach the prime of their life in the USA. CYDF
USA, (Inc) is envisioning to carry on this noble mission of providing a culturally-sensitive services in a medical model healthcare facility. CYDFI Philippines
might see some of its scholars participate in the crusade, as healthcare workers, such as physicians, registered nurses, dietitians and nutritionists,
pharmacists, therapists (OP/PT/ST), dentists, podiatrtists, psychologists, psychiatrists and social workers. The following summary table indicates where
most of the Filipinos reside in the United States, being the 2nd highest in number of Asian population:
2. THREE (3) PHASES OF PROGRAM DEVELOPMENT
CYDFI USA seeks to respond to the issues identified by strategically designing programs divided into (3) three
Phases of Program Development that would effectively in addressing the said issues, namely:

Phase 1: Community Outreach and Fundraising Drive
Phase 1 is a short-term goal that will take the task of Community Outreach and Fundraising Drive to directly benefit
the youth identified in the mission of CYDFI Philippines, and therefore, it addresses objective #1 which is designed to
directly support the mission of CYDFI Philippines, that is:
    "to empower indigent and deserving talented youth of Calantas and Nabuclod Aeta Community through a
    Scholarship Program Model (SWREM Theory In Action), designed to help them, help themselves so they
    can help incoming scholars leading to breaking their cycle of poverty, with implications not only to personal
    but community growth and development, as they learn to become socially responsible".

Phase 2: Development of Community Youth Empowerment Programs
Phase 2 is a long-term goal that addresses objective #2 which is designed to develop Community Youth  Empowerment
Programs
(CYEP) to benefit the Filipino-American "high-risk" youth and their immediate families in their search for
positive self-images, enhancement of self-esteem, development of life and leadership skills. It is a pro-active approach
of youth development evoking in them their potential for leadership and become good leaders in their own respective
communities. The objective is referred to as follows:
    "to design Community Youth Empowerment Programs (CYEP) that promote positive self-image,
    enhancement of self-esteem, development of life and leadership skills of the Filipino-American
    "high-risk" youth and their immediate families in the San Francisco Bay Area.

Phase 3: Establishment of a Nonprofit Medical Model Healthcare Facility
Phase 3 is a long-term goal that addresses objective 3 in establishing a nonprofit medical model healthcare facility to
respond on the need to provide an ethnic-specific psycho-socio-emotional, religious, and culturally-sensitive service
for the Filipino-American aging population in the San Francisco Bay Area. Phase 3 may be linked to the manpower
development of CYDFI Philippines as some of its Scholars might lean towards to the direction of becoming
healthcare workers and find themselves needed to work in the Geriatric Healthcare Center for Filipinos (GHCF). The
Center requires most of its personnel to be Filipinos with an innate capacity to provide culturally-sensitive services.
This kind of environment provides a definition of what "
Quality of Care" may mean to residents being care for. The
objective mandating this atmosphere is stated as follows:
    "to establish a nonprofit medical model healthcare facility, imbedded with ethnic-specific psycho-socio-
    emotional, religious and culturally-sensitive services for the Filipino-American aging population in the
    San Francisco Bay Area and replication advocacy of the model in other parts of the country".


3. POLICIES/PROCEDURES DEFINING STRUCTURE OF PROGRAM DEVELOPMENT
The following are policy procedural statements defining the parameters of the structure of the program development
providing direction and course of action to the Foundation:

Policy Statement #1: Sine Quod Non Habet:
Prior to determining the Policy and Procedural Statements that would define the STRUCTURES of Program
Development, and effectively carry out to accomplish the mission of the Foundation, it should be made clear that
the following
procedural statements are needed to be complied with:
1. Pre-requisite Procedural Statements:
    1a. Organization and Management.
           a. Determine from among the Calantas Folks on their commitment to come on board and join
               the crusade of the Foundation,
                                 (1) by being elected,
                           (2) as members of the board, trustees and officers,
                           (3) register the Foundation in California and secure a tax-exempt status from
                                the IRS (501.c3) and FTB,
                           (4) adopt the 3 Phases of Program Development, and
                           (5) support the mission of CYDFI Philippines by conducting fundraising activity for the
                                Phases 1 and 2 Scholarship Program in Calantas and Nabuclod Aeta Community.
            b. Agree to meet, at least, once a year for Board Meeting, but election of Officers, Board Members
                and Trustees shall be done every two years.
            c. Write and adopt the Articles of Incorporation and By-Laws of the Foundation.
    1b. Legal Requirements via Initial Agent for Service of Process
           a. Register the Foundation into a non-profit, non-stock corporation under the Nonprofit Public Benefit
              Corporation Law (Title 1, Div. 2, Part 2, ss. 5110 et seq. of the California Code of Regulations)
               through the Office of the Secretary of State in California as: CYDFI USA
           b. Obtain State Tax Exempt Status under Section 23701d of the Revenue and Taxation Code in
               California, and/or,
           c. Obtain Federal Tax Exempt status under Section 501.c3 of the IRS Code,
           d. Obtain a TIN (Tax Identification Number) from the IRS,
           e. Other legal papers, as determined and required (DOJ, BE, EDD, DIR, FTB and City).
           f. Once the Foundation is registered as a nonprofit organization, it has to be registered in the City where
               the main Office is located at: San Francisco, CA.
           g. The Foundation has to register in the City its FNSB (Fictitious Name of Small Business)
           h. The FNSB has to be legally published (legal publication)
           i. Prior to registration of CYDF USA, (Inc), its Articles of Incorporation and By-Laws are required to be
              done and signed by the members
           j. Organizational/Functional Chart need to be articulated. Create standing committees and sub-
               committees to address specific issues of the Foundation
           k. Arrive at a decision on which Bank could be officially adopted  by the Foundation.

Policy Statement #2: Funding, Financial Management and Accountability
The three (3) Phases of Program Development of the Foundation will be developed independently from each other,
but shall be under its over-all umbrella of responsibility. Each Phase shall address its own growth and development
with
Funding, Financial Management and Accountability through Audit (internal  and external). The public,
upon written request to indicate specific date and time, has the privilege to inspect the audited financial statement of
the Foundation.

2a. Funding and Financial Management Procedural Statements for Phase 1
  1. Phase 1 will have a Bank Account to be known as CYDF Scholarship Fund Account. This Account is
    dedicated to the Fundraising Efforts of the Calantas Folks residing in the San Francisco Bay Area and across
    the USA, together with Supporters and Friends of the CYDF USA, (Inc), to directly benefit the mission of
    CYDFI Philippines in Calantas and Nabuclod "in helping youth help himself  so he can help other youth
    through the Scholarship Program" leading to breaking their cycle of poverty as they learn to become socially
    responsible".
  2. Scholarship Fund Drive shall also be derived from the Fil-Am community and others through Endowment,
    Individual Financial Pledge, CES Alumni Membership Fee Drive, Sponsorship of Social and Cultural Events,
    Albertson's Community Partners Club (CYDF Handbook, Art. 7, p.40).
  3. Scholarship Fund Drive shall also be derived from annual dues, donations and gifts (CYDF By-Laws, Art. X1)
  4. Scholarship Fund Drive shall also in clude in writing grant proposals to some Foundations sensitive and
    responsive on the mission of the Foundation, both in the US and the Philippines. Information can be
    maximized coming from The Foundation Center in San Francisco, CA. on the "Guide to Funding for
    International and Foreign Programs (The Foundation Center, p40). All of the 1,300 plus grantmakers
    featured in the booklet have demonstrated a commitment to international non-profit in the US and abroad
    (Philippines). They provide funding for international relief, conference, disaster assistance, human rights,
    civil liberties, community development, education, and related subject.

2b. Funding and Financial Management Procedural Statements for Phase 2
  1. Phase 2 is a Bank Account for Community Youth Empowerment Programs (CYEP). This Account shall be
    established when Funding Grant is made available to serve primarily Filipino-American "high-risk" youth in
    their search for and promotion of positive self-image, enhancement of self-esteem, development of their life
    and leadership skills.
  2. Funding source for CYEP sha;; be derived from Foundations sensitive and responsive to the pro-active
    approach of methodology for community empowerment programs.

2c. Funding and Financial Management Procedural Statements for Phase 3
  1. Phase 3 is a Bank Account for the Geriatric Healthcare Center for filipinos (GHCF). This Account shall be
    established when the healthcare facility becomes operational.
  2. Since the healthcare facility is a nonprofit medical model, the funding source comes mostly from  the (1)
    Medicaid/Medi-CAL, (2) Medicare Part B, (3) HMO's, (4) private insurances of residents,     (5) private pay
    by residents and/or family members, (6) through linkage with SFVHA Community-Based Health Program.
  3. The Business Office of the Geriatric Healthcare Center for Filipinos (GHCF) shall take charge in implementing
    the financial policy of the facility under the supervision of the Executive Director and/or Administrator of the
    Facility.

Policy Statement #3: Implementation of CYDF Program Development
Having identified funding sources for the three (3) Phases of CYDF Program Development, the Foundation shall
address the implementation of the Programs in an incremental and longitudinal basis beginning upon completion of all
the
Legal Required Documents shall have been submitted and approved to commence its nonprofit business status
operation.

3a. Implementation Procedural Statements for Phase 1
  1. Phase 1 shall be immediately implemented and conduct fundraising activities in the San Francisco Bay Area in
    support of the mission of CYDFI Philippines.
  2. The financial resources generated by the efforts of the Calantas Folks, Friends and Supporters of the
    Foundation, shall all be directed to benefit directly the CYDFI Scholars in the Philippines (pls. refer to Policy
    Statement #2, 2a).
  3. Implementation of Phase 1 shall be strictly monitored (Bookkeeping and Annual Audit), as basic procedures of
    Financial Management.
  4. Summary of financial activities shall be reported to the members/supporters of the Foundation during Annual
    Board Meeting.

3b. Impementation of Procedural Statements for Phase 2
  1. Phase 2 shall be implemented in San francisco, CA. and the Bay Area, to benefit FilipinoAmerican youth in
    search for and promotion of positive self-image, enhancement of self-esteem, development of their life and
    leadership skills.
  2. Full implementation shall depend on support (Funding of Grant Proposals) coming from Foundations sensitive
    and responsive to the pro-active approach of methodology in empowering youth to function at best in their life
    through "Community Youth Empowerment Programs" of the Foundation.
  3. The Foundation shall have to design and develop "content and process" of Youth Empowerment Curriculum
    that addresses (1) promotion of self-image, (2) enhancement of self-esteem, and (c) development of life and
    leadership skills.
  4. Personnel Development to carry out Phase 2 is subject to the pulse created in the CYDF-CYEP Grant
    Prposal's approval for funding.
  5. The Foundation will seek to engage youth in San Francisco who were already engaged during their elementary
    and middle school education about youth's involvement in the community and could be resources to provide
    feedback on their "thoughts and feelings as Filipino youth versus being a Filipino-American youth" in the USA
    today. How do they see themselves? How good they feel about themselves? Do they know who they are, as a
    people?

3c. Implementation Procedural Statements for Phase 3
  1. Phase 3 shall be implemented in the San Francisco Bay Area and advocate for the replication of the model in
    other parts of the country where there is a higher concentration and large number of Filipino-American aging
    population with the establishment of a nonprofit medical model healthcare facility, imbedded with ethnic-
    specific psycho-socio-emotional, religious and culturally-senstive services.
  2. The actual implementation shall be based on the Field Study that the CYDF Founder submitted to the Graduate
    School in Gerontology at San Francisco State University last May, 2005. A full discussion copy of the Field
    Study shall be attached in preceeding presentation.
  3. As a Foundation with nonprofit status, CYDFI USA may experience a tax-exempt status that will benefit the
    target population it plans to serve and those individuals, who would join the crusade.
  4. What is a skilled nursing facility? Per CCR Title 22, Div. 5, Ch.3, Art. 3, on Licensing and Certification of
    Health Facilities and Agencies, states that:
    "Skilled nursing facilities shall provide, but shall not be limited to the following required services:
    physician, skilled nursing, dietary, pharmaceutical and activity program".
"Helping indigent but deserving talented youth in Calantas/Nabuclod
through a Scholarship Program so they can help incoming scholars
leading to breaking their cycle of poverty" is a challenge that
Calantas Folks would like to accept and help to confront.
